Day Habilitation Services

Modest Blended Day Habilitation Center in New Dorp

MODEST Blended Day Habilitation Program offers two types of habilitation services: Day Habilitation and Pre-Vocational. Both services give a person with developmental disabilities the opportunity to be productive in the community. Services are delivered through a variety of structured training sessions at MODEST Day Habilitation Center.

Services are also delivered in the community, where individuals engage in a variety of social and independent common life skill training sessions, recreational activities, and pre-vocational activities by volunteering at various sites to develop new skills. The program is offered five days a week, Monday through Friday 9:00am – 3:00pm, and offers daily transportation service for all individuals attending the program. Both Day Habilitation and Pre-Vocational are (HCBS) Home and Community Based Services-Waiver federally approved.

Day Habilitation without Walls

MODEST Day Habilitation without Walls provide supports and services to individuals with developmental disabilities that are necessary to motivate them to achieve greater participation within their communities. The program activities are designed to inspire, assist and support each individual to have a meaningful role which includes the ability to care for one-self and to increase productivity and competence. Individualized supports and services are commensurate with the person's strength, capacities, interests and desires that are consistent with each Individualized Services Plan. The program is offered five days a week, Monday through Friday 9:00am – 3:00pm, and offers daily transportation service for all participating

Scope of Services:

The structured support sessions at the Day Habilitation without Walls includes, but are not limited to:

Socialization Skills:
  • Maintaining interpersonal relationships, understanding self advocacy and the ability to respond to safety and health problems accordingly.
  • Participate in internet usage; and learn reading, writing, and math skills that are applied in everyday life.
  • Exercising choices, initiating planning activities and problem solving.
  • Communicating with others, understanding emotions and social cues and having the opportunity to enjoy and experience the New York City's social, cultural, and entertainment resources.

Vocational and Employment Skills:
  • Ability to maintain full-time or part-time employment, either competitive or sheltered, follow instructions, cooperate with peers and staff, task completion and meets work standards.
  • Taking responsibility for one's own activities and having the ability to participate in volunteer opportunities in the community.
  • Understanding community services such as using public transportation and the ability to respond to safety issues.
  • Utilize the use of the community resources to the maximum to increase individual's productivity and competency.
